A study was carried out in a village (Ahamansu) of about 500 people in the Volta River basin of Ghana in an attempt to identify infection-free "putatively immune" (PI) individuals in thisOnchocerca volvulushyperendemic area. Participants were life-long residents of the village, which had a prevalence of parasitologically determined microfiladermia of >90%. Potential PIs were evaluated for onchocerciasis on 3 separate occasions over 2 consecutive years. On each occasion, clinical evaluation consisted of a medical history, physical examination, examination by a consultant dermatologist, palpation for nodules, slit lamp examination and 6 skin snips for parasitological examination. As a result, 10 subjects were identified as PI. Skin snips from these 10 individuals were then subjected to PCR forO. volvulusDNA. 9 of them hadO. volvulusDNA present in the skin, indicating current infection with the parasite. Therefore, after a laborious search in this village, only 1 person was truly (by current state of the art criteria) infection-free.
