CONCLUSIONChronic metabolic acidosis in CKD is associated with increased morbidity and mortality. Correction of acidosis not only can alleviate the renal injury and fibrosis but may also protect other organ systems frequently impaired in the course of CKD. Rigorous long-term randomized trials designed to test the optimal time of therapy initiation and target serum bicarbonate level are eagerly awaited.
