To the Editor. —Orbital involvement in mycosis fungoides, a form of cutaneous lymphoma, is rare. We describe a patient with mycosis fungoides, in whom the pattern of orbital involvement was demonstrated on computed tomograpy (CT). Report of a Case. —Over a ten-day period, a 70-year-old woman with mycosis fungoides (diagnosed by means of a skin biopsy four years earlier) had progressive proptosis, conjunctival chemosis, and ophthalmoplegia. She also had a reduction in visual acuity to 20/800 first in the right eye and then the left eye. During the previous year, she had been treated with topical corticosteroids for recurrent iritis in both eyes. Computed tomography, performed after the development of right-sided proptosis, demonstrated marked enlargement of the right medial rectus, inferior rectus, and optic nerve (Figure). Treatment with dexamethasone (48 mg/day) rapidly reduced the conjunctival chemosis and proptosis and improved visual acuity to 20/40 OS, although it remained 20/800 OD.
