A review of the assessment of obesity, whether in children or adults, typically begins with the dictionary definition of obesity as “excessive fatness.” This leads to consideration of the assessment of fatness and of the determination of the normal range beyond which the amount of body fat is excessive. The measurement of body fat is indirect at all ages and based on assumptions that are approximate at best and commonly erroneous. Furthermore, because errors are associated with all measurement procedures, the numerical values obtained are less accurate than they might appear. The limitations implied in the preceding sentences are marked for children in regard to both the assessment of obesity and the determination of the normal range.This review will be restricted to total body composition and to methods based on the following:(i) total body water,(ii) total body potassium,(iii) skinfold thicknesses,(iv) electrical conductivity,(v) bioelectric impedance,(vi) photon absorptiometry,(vii) body density, and (viii) predictive equations. The emphasis will be on the validity of the assumptions underlying these methods when they are applied to normal children rather than the practicality of their application.
