Furthermore, studies of secondary prevention of cardiovascular disease are needed to confirm whether the predictive value of HDL cholesterol concentrations for cardiovascular risk are diminished in the presence of very low LDL cholesterol, and whether other lipid measurements might offer greater predictive accuracy of cardiovascular risk. The findings from today’s study should not detract from the fact that raising HDL cholesterol remains a major treatment strategy for the reduction of cardiovascular risk in the large majority of patients who do not have very low LDL cholesterol; 12 the problem, in most cases, is how to achieve this strategy.Lifestyle changes, such as more aerobic exercise, weight loss, smoking cessation, moderate alcohol intake, and dietary changes might result in modest increases in HDL cholesterol. 12, 13 Drugs that specifically raise HDL cholesterol concentrations are less available, with niacin being the most effective current agent. Others include the fibrates, the thiazolidinediones, and the glitazars. 12, 13 However, two new inhibitors of cholesterylestertransfer protein are in clinical testing (anacetrapib and dalcetrapib), and they might substantially increase HDL cholesterol without the off-target effects observed with torcetrapib. 12, 13 Therefore, with the advent of more potent drugs, the issue of whether raising HDL cholesterol concentrations reduces cardiovascular risk in patients with very low LDL cholesterol needs to be tested in clinical studies.
