Penile lentigo refers to localized asymptomatic macular hyperpigmentation without evidence of histologic atypia. The incidence of this disease is not known although 14.2% of 10,000 males between the ages of 17 and 25 were reported to have pigmented nevi. Authors report a case of penile lentigo, the histologic examination of which showed diffuse melanocytic hyperplasia with focal atypia. A 25-year-old white man presented with an asymptomatic pigmented lesion on the glans penis of 2 years duration. The lesion had been slowly increasing in size. There was no history of any drug intake, trauma, or prior eruptions in that area. On examination, the patient showed macular hyperpigmentation of tan, brown, and black color variations irregular in outline with skip areas.
