We have been interested in studying the immunological and clinical specificity of immunochemical methods for the determination of prostatic acid phosphatase levels in patients’ sera. Our reasons for this interest are: (1) Immunochemical methods offer the promise of providing greater specificity for the measurement of prostatic acid phosphatase compared to catalytic assays. No substrate for the determination of catalytic acid phosphatase activity has been shown to be absolutely specific for the prostatic isoenzyme of acid phosphatase. (2) Since we in the clinical laboratory have the responsibility to interpret abnormal levels of prostatic acid phosphatase in patients’ sera in collaboration with physician staff, we need to define rigorously all possible sources of elevated immunoreactive prostatic acid phosphatase. To this end we have studied (a) the distribution of immunoreactive prostatic acid phosphatase in normal and diseased human tissues with an immunoperoxidase technique; (b) the cross-reactivity of several purified preparations of acid phosphatase from nonprostatic tissues with antiprostatic acid phosphatase antiserum by a radioimmunoassay technique; (c) the possible presence of immunoreactive prostatic acid phosphatase in extracts of nonprostatic tissues and blood cells, and in the serum and urine of women; and [d] the quantitative levels of immunoreactive prostatic acid phosphatase in the serum of male patients who do not have adenocarcinoma of the prostate, in order to determine the clinical specificity of this procedure.
