Vasospasm without subarachnoid hemorrhage in an unoperated aneurysm is a rare event not easily explained by current theories. A report of such a case is presented. A plausible explanation involving recent identification of vasoconstrictive prostaglandins (prostaglandin endoperoxide and thromboxane A2) and the opposing action of vasodilator prostaglandins (prostacyclin) is offered.
