Dr. Stjernfeldt and colleagues reported a higher frequency of in-utero exposure to maternal smoking among children with cancer as compared with juvenile diabetics in Sweden. Maternal smoking was associated with a doubled risk of 3 tumors with seemingly diverse etiological (epidemiological) features--namely kidney tumor non-Hodgkin lymphoma and acute lymphoblastic leukemia (ALL). The finding is based on small numbers and conditional on diabetics being an appropriate control group. The frequency of maternal cigarette smoking during gestation of the 280 Swedish children with cancer has been compared with our interview data for 436 childhood cancer cases in the US (table). We did not examine smoking "dose" and the basis of the comparison is any reported maternal smoking. Exposure rates in the 2 series are similar for all tumors combined but discrepant specific tumor types. As compared with the US exposure to maternal smoking in Sweden was much more frequent among children with Wilms tumor non-Hodgkin lymphoma and ALL. The opposite was reported for Hodgkins diseases. When the 2 series are merged maternal smoking is not associated with excess risk of these types of tumor. Smoking during pregnancy can certainly harm the developing fetus but its role as cause of childhood cancers remains uncertain. (full text)
