The diagnosis of a malignant neoplasm such as a malignant melanoma is a “dramatic event,” not only for affected individuals but also for physicians who suspect the condition clinically, have it confirmed by a pathologist, and are responsible for management. In this setting, clinical attention is naturally directed away from the antecedents of events of the lesion to consideration of future events, namely, thc management of the neoplasm and the natural his-tory of the disease following “definitive” therapy. In the case of a malignant melanoma, a rich array of prognostic variables is available to assist in deci-sions about management and in assessing prognosis. lt should come as no surprise to learn that all ma-lignant melanomas do not carry the same sinister significance, yet this information has evolved slowly in the last 20 years. Only very recently has there been wide acceptance of the idea that some mela-nomas are very unlikely to recur and metastasize, and that their management can and should be modificd accordingly."?
