ISTORICALLY, the therapeutic application of ultrasound was first proposed by Freundlich in 1932 [l], with the first report of its application to an animal tumor following closely in 1933 by the noted biophysicist Albert Szent-Gyorgyi [2]. Further development of the necessary piezoelectric technology led to the first publication of application to humans for the purposes of physical therapy by Pohlman et al. 131 in 1939. Since that time, the application of ultrasound for physical therapy has enjoyed wide popularity and continued growth throughout the world. The first description of the treatment of a human tumor came in 1944 when Horvath [4] reported on the treatment of a sarcoma. An excellent review of progress in ultrasound cancer therapy to 1979 has been written by Kremkau [S]. As of 1979 there were only four institutions engaged in human cancer treatment worldwide using ultrasound, and only two in the United States [S]. The rather slow development of ultrasound for cancer treatment probably resulted from several factors. First, early reports from both animal and human studies reported conflicting and rather pessimistic results when ultrasound was employed as single agent therapy. Pohlman [6] undertook a survey of the literature prior to 1950 and showed that of the 133 cases reported only 17 percent derived some benefit. This poor clinical performance led to the “Erlangen resolution” of 1949 that in essence stated that the use of ultrasound in clinical cancer treatment should be abandoned.
