The authors report a fatal case ofBrucellasuis endocarditis. The patient was employed as a meat cutter in a park packing house. Although endocarditis due toBr. abortusandBr. melitensisis well known this case is important both because of the rarity ofBr. suisendocarditis and the lack of evidence of any previous damage to the heart valves. There was no history of acute rheumatism and at the onset of the illness no evidence of a cardiac abnormality. Blood cultures at the time of the initial illness remained sterile but brucellar seroagglutinins were present to a titre of 1 in 5120. Initially the patient responded, apparently satisfactorily, to 21 days treatment with tetracycline and streptomycin but 2 months later there was evidence of cardiac involvement andBr. suistype 3 was recovered from 2 of 5 blood cultures. The patient responded to a combination of antibiotics and sulphonamides but after working for 3 months as a truck driver he relapsed and, in spite of intensive therapy and the replacement of the damaged aortic valve by a prosthesis, death ensued on the 38th post-operative day. Post-mortem examination showed that mycotic growths occluded the prosthetic heart valve. Treatment and the possible role of dental sepsis are discussed.Frederick J. Wright.
