Much of relevance to fertility, pregnancy, growth and development has changed in the 25 years since the Society for Pediatric and Perinatal Epidemiologic Research was founded. Entire fields of study that hardly existed, such as the Developmental Origins of Health and Disease, have blossomed. Autism, once considered an obscure rarity, is now recognised to be distressingly common; and obesity went from being not rare to reach epidemic, even crisis proportions. Although epidemiological investigators, on their own and in collaboration with clinical and laboratory investigators, have learned much about the mechanisms underlying reproductive disorders, most putative reproductive risk and protective factors that were under investigation when the Society for Pediatric and Perinatal Epidemiologic Research was founded are still under active investigation. Over the past 25 years few have been conclusively proven or disproven to be causal and, with the exception of folate for the prevention of neural tube and perhaps other birth defects, our advanced knowledge has by and large not translated into reduced occurrence of adverse events. Some of the pressing questions that will pass on to the next generation of scientists relate to nutrition, preterm birth, preeclampsia, folate, environmental hazards and the role of biostatistics.
