ACQUIRED DISTURBANCES of reading-alexiahave traditionally been divided into two categories: the inability to read words and the inability to read letters. Although this verballiteral separation of alexia has consistently received mention, there has been little to suggest separate disorders. In almost a century of use, this separation has not proved to have significant clinical usefulness and there has been no demonstration of neuroanatomical correlation. Most authors now consider word and letter reading as points on a continuum, some cases showing relatively greater difficulty in one aspect than the other but almost all demonstrating both. This paper presents a series of patients simultaneously resident on the Aphasia Research Service of the Boston Veterans Administration Hospital who clearly illustrated this verbal-literal dichotomy and further suggested a n anatomical correlation. Some implications of these findings on the theories of reading disturbance will be discussed.
