GSZS reproduction is immuno-logically unique. In normal preg-nancy the maternal immune system func-tions perfectly well in protecting the mother from invasion by foreign organisms, yet it fails to reject the antigenically foreign fetus and placenta. This paradox has intrigued investigators for years. In light of the mystery surrounding the immunological acceptance of the conceptus, it should come as no surprise that immune dysfunction has been blamed for preg-nancy loss in some patients. Essentially, the supposed immunologic causes of pregnancy loss fall into three categories:(1) alloimmune: the failure of the develop-ment of the normal maternal tolerance to pregnancy, with subsequent immunologic rejection of the conceptus,(2) isoimmune*: destruction of fetal erythrocytes by maternal antibodies directed against fetopaternal red cell antigens, and (3) autoimmune: damage offetal or uteroplacental tissues by maternal autoantibodies directed against maternofetal antigens.
