The association between abnormalities of the brain in the posterior fossa and syringomyelia is becoming well known. It is suggested that "communicating syringomyelia" is a convenient term to describe those cases in which there is a communication between the syrinx and the fourth ventricle. Attempts to explain this association have usually involved concepts of cerebrospinal-fluid flow and the arterial pulse waves transmitted to, and pulsating through, that fluid. It is suggested that pressure changes secondary to venous-pressure swings, taken in conjunction with the abnormalities of the posterior fossa and upper cervical cord, explain the findings more convincingly. It is suggested that partial blockage of the subarachnoid space in the region of the cisterna magna is responsible for cerebrospinal fluid being directed into the communication, thereby providing an intermittent distending force which may be active for many years.
