The Whiplash ImPaCT trial from Rebbeck et al. 11 published in PAIN describes an ambitious undertaking with many intriguing elements: a timely and important clinical question, findings from a multicenter trial, strong training and implementation components, and faithfully followed standards for trial reporting. The team enrolled and randomized 216 participants with acute whiplash who were stratified for risk of poor outcomes and treated with risk stratified or usual care. No between-group differences were found for the primary outcomes, and the trial team reported that baseline risk status was not a treatment effect modifier. Rebbeck et al. 11 present a thorough and balanced view of trial results that includes multiple credible reasons for their findings. So, instead of reiterating those messages, this Commentary will focus on how the trial fits into existing evidence of risk-stratified care for musculoskeletal pain. Indeed, there now exists a robust trial record testing the efficacy or effectiveness of risk-stratified care over the past 12 years. Publication of the Whiplash ImPaCT Trial in PAIN provides the appropriate vehicle to reflect on what have we learned during this time.
