Graft-versus-host disease (GVHD) consists of a group of clinical manifestations and histopathologic findings that are provoked by what is termed the GVHD reaction (GVHR). This reaction is due to the immune competent cells of a transplanted tissue (the graft) reacting with the tissues of an immunosuppressed, histoincompatible recipient. The skin is a primary target organ for GVHD, and recognition ofthe cutaneous symptoms invoked by GVHD is important for at least three reasons: (1) Tbe cutaneous signs are sometimes the first symptoms of a GVHR. They may be an isolated or a predominant clinical finding or may be associated with other clinical signs. The recognition of cutaneous signs can facilitate a prompt histological confirmation of the diagnosis and rapid treatment. (2) Sometimes the cutaneous manifestations of GVHD themselves are so severe that they can be responsible for considerable morbidity and mortality (eg, toxic epidermal necrolysis, TEN) or gross functional sequela, such as sclerosis. (3) The cutaneous manifestations of GVHR may mimic other dermatoiogic disorders, such as lichen planus or TEN. Therefore, GVHD may serve as a biological model that may provide some insight into the pathomechanisms of other skin diseases.
