Down Syndrome is a severe birth defect that is most commonly caused by trisomy of chromosome 21 16 and affects about 1 in 700 live births. Although the associated developmental defects and cognitive 17 function have received the most attention, cardiovascular complications are the number one cause of 18 morbidity and mortality in Down Syndrome patients, where up to 50% of patients suffer from 19 congenital heart diseases. 2021 Emerging evidence has highlighted the role of sex as a modifying factor of heart diseases, 22 including in age associated heart diseases such as heart failure with preserved ejection fraction which 23 afflicts women more than men. 2 Information on congenital heart diseases remains less well understood. 24 Prior studies have noted potential sex differences in congenital heart disease risks in male and female 25 pediatric Down Syndrome patients; however, some studies have reported contrasting results that may 26 reflect effects of specific cohorts and geographical regions, hence a systematic picture is needed which 27 will be important for informing ongoing gaps and outlining pressing research questions in the field. 28 29
