Surgical exposure of all or any part of the facial nerve in its course through the temporal bone is often necessary. The ideal procedure allows exposure of the involved portion of the facial nerve without causing further injury to the nerve, to hearing, or to vestibular function, and without producing a permanent mastoid cavity. Lewis 1 described a method for exposing the facial nerve from the stylomastoid foramen to the geniculate ganglion, through the facial recess, without disturbing the posterior bony wall of the external auditory canal or the ossicular chain. Using an approach through the middle cranial fossa, Clerc and Batisse 2 in 1954 placed a graft in the petrous portion of the facial nerve. Panse 3 in 1904 proposed a translabyrinthine approach to exposure of the internal auditory canal. House perfected the middle-cranial-fossa approach 4 in 1961, and the translabyrinthine approach 5 in 1963. He stressed the importance
