TOL, 19 IN lEstimating the economic cost of specific diseases, illnesses, and injuries has bccome a matter of great interest to both public and private medical decision makers in recent years. Thc dramatic rise in applications of cost-benefit and cost-effectiveness analyses over the past ten years in both the medical and nonmedical literature [8] reflects the deepening concern over cost escalation in the health care sector. With US expenditures for health care services estimated to have approached $224 billion in 1980 [2] and the rate of increase in medical care prices estimated to be in the range of 10 percent or higher, policymakers have begun to look more seriously at prevention of illness and injury as an attractive strategy not only to reduce mortality and morbidity but also to contain health care costs. While improvements in health outcome used to be the bottom line in medical decision making, consideration of costs along with effec-tiveness has become an important dimension in evaluation of medi-cal practice and treatment.
