IT IS USUALLY stated that certain substances of small molecular size can penetrate easily into the brain tissue from the cerebrospinal fluid, although they cannot pass the blood-brain barrier. According to Bakay,l this penetration is possible only when the particle radius of the substance is less than 10 A. However, Bowshe9 found that large molecules of radioactive proteins could enter the brain tissue along the blood vessels and also observed a slight, diffuse radioactivity in the subpial tissue. Mandelstamm and Krylow3 considered that the penetration of substances from the cerebrospinal fluid into the brain was due to the flow of cerebrospinal fluid in the Virchow-Robin space. Davson' believed that simple diffusion through the mesothelial lining of the subarachnoid space and through the ependyma of the ventricles played a significant part. Bakayl85 noticed that penetration through the ependyma was dependent on changes in cerebrospinal fluid pressure. He found that withdrawal of cerebrospinal fluid followed by intracistemal injection of radioactive phosphorus (P32) gave rise to radioactivity in the ependymal lining and that, after identical injection without withdrawal of cerebrospinal fluid, no radioactivity was seen there. It can be added that some investigators6 have considered that the apparent difference in the penetration of substances into the brain tissue from blood and cerebrospinal fluid may be due only to the inappropriate selection of the dosage, which was considerably higher when the cerebrospinal fluid route was used.
