Neutral, is osmotic Na-K phosphate, 1-3 mM/kg, was infused over 6 hr. in 6 normal, 3 treated hypoparathyroid, and 4 hyperparathyroid subjects, and in 3 patients with cancer and hypercalcemia, all on constant diets. Serum [Ca] in all during P infusion. Urinary and fecal Ca excretion also fell; thus Ca was not lost from the body. The fall in serum [Ca] was linearly related to the mean [Ca x P] mole product during infusion: A Ca mM/L = 1. 26-0. 28 [Ca x P] mole x 10-6 (r = -0. 85). The percentages of serum [Ca] and [P] which were ultrafilterable did not change. Thus, since the [Ca x P] mole product is a measure of the tendency for Ca-PO4 salt formation, the infusion of P appears to lower [Ca] by a physico-chemical mechanism. [P] returned to control levels within 24 hr. in all subjects. By contrast, [Ca] remained reduced for days in the patients with hypercalcemia, long after [Ca x P] had declined to control. [Ca] returned to control within 24 hr. in the normal subjects. Thus the rate of restoration of [Ca] appears unrelated to [Ca x P] and involves some other mechanism, possibly parathyroid hormone, since recovery of [Ca] was also delayed in the hypoparathyroid subjects. || ABSTRACT AUTHORS: Authors
