single-agent adjuvant (prophylactic) chemotherapy after surgery have provided only minimal encouragement and often conflicting results." Now that more active chemotherapy is being defined and a greater understanding ofthe requirements of adjuvant chemotherapy is available, properly conducted trials evaluating this approach in patients at high risk after apparently curative surgical resection are clearly indicated. A multi-institutional trial of adjuvant chemotherapy for gastric cancer is also being undertaken in Australia, where the combination of 5FU, doxorubicin and BCNU is compared with no subsequent 'therapy after surgery. If the potential for this combination in the advanced setting holds true in the earlier stages of disease, then the proportion of patients relapsing after initial definitive therapy should significantly decline. However, it is important that these trials be conducted properly and with caution, as these combinations are potentially toxic and success is by no means guaranteed. If these studies confirm the promise of chemotherapy, then the future for patients with gastric cancer will be one of considerably greater optimism.
