Introduction"/'he dramatic advances made in the management of testis germ cell tumors in the last decade are well known. Quite realistically, virtually all patients with early stage disease (6) and about 60-70% of patients with disseminated disease (2) can be cured by modern therapy. However, there remains a significant proportion of patients with metastatic disease who fail to be cured. Thus, investigations of secondary treatment regimens are appropriate.Prior to 1978 at Indiana University, patients who failed to attain complete remission with 4 courses of c/s-platinum+ vinblastine+ bleomycin:: i: adriamycin (PVB±A) or those who relapsed from complete remission were treated with a variety of secondary c/splatinum-containing regimens. Although most attained a further objective response, there were no cures. At the same time, we saw a small group of patients who had failed prior vinblastine+ bleomycin. They were treated with cis-platinum+ adriamycin (3). Objective responses were frequent, but no patient experienced long disease-free survival.
