Relationship between serum angiopoietin-2 level and perfusion CT paramet

To the Editor: Early prediction of complications of severe acute pancreatitis (SAP) such as multiple organ failure and development of pancreatic necrosis is essential to improve the clinical outcome of patients with SAP. Effort has been made to establish methods to predict complications of SAP. Perfusion computed tomography (CT) is a promising technique to predict development of pancreatic necrosis (1, 2). Recent study by Whitcomb et al.(3) showed that elevated serum angiopoietin-2 (Ang-2) on admission is predictive of persistent organ failure in patients with SAP. Ang-2 is produced by damaged vessels and increases vascular permeability (4). Role of elevated Ang-2 in development of organ failure and relationship with pancreatic necrosis is unclear. Therefore, we investigated the relationship between serum Ang-2 levels and perfusion CT parameters, surrogates of local and systemic circulation, in patients with SAP (1, 2).This study was approved by our institutional review board. All patients had given written informed consent. From November 2004 to October 2008, 11 patients with SAP whose serums were collected on admission and who underwent perfusion CT on admission were enrolled. Severity of acute pancreatitis was assessed by the Atlanta criteria and/or Acute Physiology and Chronic Health Evaluation II scores. Serum Ang-2 levels were measured on the stored sera in a retrospective manner by enzyme-linked immunosorbent assay.
