We recently reported a colorectal cancer (CRC) subtype transcriptional classifier that defined five subtypes using published transcriptional profile data sets, and we suggested an association between certain subtypes, including the ‘stem-like’subtype, and response to first-line FOLFIRI treatment1. These associations were based on analysis of transcription profiles from a published study2 (referred to as the Del Rio data set) of 21 first-line FOLFIRI-treated patients and on analysis of the responses of eight CRC cell lines to treatment in vitro with a combination of irinotecan and 5-fluorouracil (key components of the FOLFIRI treatment protocol).Martinez-Garcia et al. 3 take issue with the reported association between subtype and FOLFIRI response, noting that the association we reported was not statistically significant (P= 0.18) and was not corroborated by their analysis of ten additional patients. They conclude that “it is likely that to obtain an association between a subtype and response to treatment, a larger data set is needed.” We agree with that assessment. Indeed, it was the small sample size and the not quite significant association of the stem-like subtype with FOLFIRI response that prompted us to assess response to FOLFIRI components in vitro using CRC cell lines. Our suggestion of a possible association between stem-like subtype and FOLFIRI response was based on the combination of clinical and biological response associations.
