Fractionated carbon dioxide (CO2) laser resurfacing has become a popular surgical laser procedure for facial rejuvenation and acne scarring. It is associated with less downtime and patient discomfort than traditional full-face ablative laser resurfacing. Prolonged erythema, pigmentary changes, and infections are rare potential complications after CO2 resurfacing and have been reported, but these sequelae may occur with less frequency during fractionated procedures. In the case report that follows, we describe the first known reported case of an atypical mycobacterial infection (Mycobacterium chelonae) after fractionated laser resurfacing, necessitating dual therapy with clarithromycin and linezolid.
