In order for a treatment modality or pharmaceutical agent to be properly evaluated in a clinical trial, a logical, objective endpoint must be clearly defined and easily tested. In medicine, the endpoint evaluated is often a reduction in the incidence of a specific disease. Unfortunately, in the case of dental diseases such as caries and periodontitis, such endpoints are difficult to monitor primarily because these oral diseases often take a prolonged course. Thus, it is often impractical to conduct clinical trials over a sufficiently long period of time to assess the effects of an intervention on the incidence of oral disease. On the other hand, since these oral diseases are known to be the result of the action of dental plaque, it is reasonable to monitor aspects of plaque formation and development to determine the potential clinical efficacy of a specific therapeutic method or agent. This chapter briefly reviews the role of plaque and specific microorganisms in the etiology of caries and periodontitis and describes and evaluates critically the various methods available to monitor plaque and plaque microorganisms over time. The usefulness of these methods in monitoring the efficacy of plaque control methods is discussed in the context of the nonspecific or specific plaque hypotheses.
