Introduction Since the 1986 appearance of crack, an impure freebase form of cocaine, an increasing numberof complications associated with crack use have been described. In the head and neck, thermal injury due to inhalation of hot vapors, crack particles, or filter screens has been well described.These injuriesincludeoralulcerations,1uvular edema and paradoxic vocal fold motion," and edema, ulceration and necrosi s of the oropharyngeal, hypopharyngeal and laryngeal mucosa."? Ingestion of a filter screen with subsequent esophageal thermal injury has also been reported." Crack can be smoked by rolling the particles into a cigarette or by heating the crystals in a home-made pipe. In a "shotgun" technique, a second individual forcibly blows the cocaine vapors through a special pipe into the user's mouth, rapidlydelivering a greater amountof drug than inhalation alone. Although no one has measured the intraoral pressures this maneuver creates, the patient reported in the present case noted that it wouldoftencause her ears to pop. We present a case of retropharyngeal emphysema and abscess which may have resulted from "shotgunning" crack cocaine.
