Potent antiretroviral therapy stimulates robust and durable immune reconstitution among profoundly immunosuppressed patients that is clinically important. Fifteen patients with cytomegalovirus retinitis (CMVR) were followed after discontinuing anti-cytomegalovirus therapy. No patient had progression or relapse for a median of 51 months. Survival from the diagnosis of CMVR was universal for up to 95 months, with an observation period averaging more than 6 years. The CD4 cell count continued to rise through the fourth year of follow-up.Before the widespread use of highly active antiretroviral therapy (HAART) in developed countries, cytomegalovirus retinitis (CMVR) was a significant cause of morbidity and a predictor of mortality. Recent studies have demonstrated that chronic maintenance therapy may be safely discontinued among patients with CMVR whose CD4 lymphocyte counts have demonstrated a sustained increase to more than 50–150 cells/mm 3 in response to HAART [1–6].
