Much controversy has centered on the relative advantages of enteral and parenteral feeding of the acutely ill patient. Enteral nutrition offers a major advantage, with its ease of administration and theoretical nutritional efficacy, given the more'physiological'clearance of nutrients. Clearly, the integrity of intestinal mucosa is best maintained by enteral feeding. Recent evidence suggests that the mucosa of the gut may play a greater role in immune function than was hitherto realized and may offer a first line of defense in the critically ill patient. The aim of the present discussion is to review some Of the more recent studies in this area and new evidence suggesting that immune function can be modulated by enteral nutrition.
