interviews were conducted in one large UK PICU with doctors, nurses, pharmacists and managers. These were audio-recorded, transcribed and analysed using Burnard’s Framework (1991)1. This research was reviewed and approved by UCLan Ethics Committee, all participants signed written, informed consent. Results: Team members articulated their perceptions of medication errors which were characterised by four overlapping and re-emerging themes: PICU culture; inconsistencies and misconceptions; learning and engagement; and how we evaluate and feedback learning and improvements. Conclusions: The data provides an understanding of how health professionals on PICU perceive medication errors. Staff engagement should be sought at all levels to promote learning.
