However, the rates shown are lower than previously reported.2.3 This reduction may reflect improvements in early recognition and treatment of reactions, as well as a decrease in errors resulting from improved quality assurance programs, enforcement of policies, and increasingly automated identification procedures. The ongoing risk, however, emphasizes the continued need for the transfusion medicine community to work to eliminate preventable deaths due to ABO-incompatible transfusions.
