Decreased coronary blood flow in ischaemic heart disease is primarily mediated by the formation of atherosclerotic lesions. Atherosclerosis may be observed in most large and medium-sized arteriesin man, withafrequency increasing with age. It may be a truism but still needs emphasizing, that the key to the pathogeneticeffectsof atherosclerosisisthe volume of the atherosclerotic lesion. The tissue mass thatrestrictsbloodflo wis composed ofthreemajor components;(i) cells;(ii) connective tissue; and (iii) lipids. A s each one of these components make a substantial contributon to the volume of the atherosclerotic lesion, it would be unrealistic to try to explain the significance of the lesion withouttakingallthree components into account. One ofthemajorproblemswit h many of the hypotheses proposed for atherogenesis has been tendencies towards one-sidedness. I n the present review, we will examine each one of the components first, then try to make a synthesis concerning what events might have lead to (i) an increased cell number;(ii) connective tissue synthesis; and (iii) lipid deposition.
