Compliance as a health care issue is of concern to all medical, nursing, and related health care disciplines because the outcome of therapy in many cases is dependent on the patient/client taking an active role in his care. The purpose of this paper is to present an overview of the major issues of compliance in health care, to offer some practical suggestions for improving compliance, and to discuss some aspects of research methodology in compliance. Concluding remarks will focus on some provocative research questions that hopefully will stimulate nurses to become involved in compliance research.In health care literature, compliance can mean a variety of things. In general, it means following the recommendations or prescriptions of a health care provider. It can mean keeping appointments, following a diet, or taking medications. Some in the field of compliance prefer to use the word adherence instead, as this term implies a less coercive role on the part of the care provider and a more active, cooperative role on the part of the patient. Compliance, however, is the most commonly used term. Compliance with medical therapies requires that the person change his lifestyle in some way–either by adding a behavior (taking pills, exercising) or omitting a behavior (salting food, eating sugar, drinking alcohol). Change for many people is difficult. It is not surprising, therefore, to find 30 to 60 per cent (some figures go as high as 90 per cent) of people do not comply or adhere to a prescribed treatment or rec-
