Although smoking is on the decline in developed nations, it is increasing at an alarming rate in developing nations, including Guatemala [1–4]. There, the tobacco control movement is only beginning to take hold, with physicians poised to help lead the way. Physicians can be strong community leaders by encouraging smoke-free environments, treating nicotine addiction, and serving as non-smoking role models [5–9]. However, in order for physicians to be effective, they must be knowledgeable about the health consequences of tobacco, as well as act on that knowledge. This is particularly important in Guatemala and other developing countries, where tobacco may not be a focus in medical training. To evaluate the knowledge and use of tobacco among physicians in Guatemala, we surveyed medical residents in the country’s two largest teaching hospitals, Roosevelt Hospital and San Juan de Dios Hospital. In January 2002, we asked residents in the internal medicine, pediatrics, surgery, and obstetrics/gynecology departments to complete self-administered questionnaires on their knowledge, attitudes, and practice related to tobacco use. We used a modified version of a previously validated questionnaire on smoking behaviors and also included additional questions about the health consequences of smoking and secondhand smoke [10]. We collected information from a total of 174 residents (64% response rate) who ranged in age from 23 to 37 years.
