This is a report, from San Francisco, of a homosexual man aged 34 years who died fromPneumocystis cariniiinfection after failure to respond to treatment with co-trimoxazole and pentamidine. Electron microscopy revealed the cystic form ofP. cariniiin the ganglion and inner plexiform layers of the retina in the vicinity of retinal vessels. During life, multiple cotton-wool spots had been seen on the retina. It is suggested that the presence of such spots in homosexual men without other evidence of ischaemic retinal disease should alert physicians to the possibility of infection withP. carinii. F.I.C.
