Despite thorough investigation according to various clinical protocols, the underlying cause remains unexplained in about 50% of pre-term deliveries. The role played by endocrinological factors such as increasing androgen concentration, hyperprolactinaemia and luteal phase defect are thought to be important in the onset of spontaneous parturition, especially among those pregnancies that end in the second-or early third-trimester, resulting in very-lowbirth-weight neonates (Bibby and Stewart 2004). Spontaneous parturition in primates is characterised by fetal adrenal synthesis of c19 androgens, which, in turn, are aromatised by the placenta into oestrogens (Bibby and Stewart 2004). Exogenous androstenedione infusion produces premature delivery in pregnant monkeys (Mecenas et al. 1996). Gravett et al.(2000) presented evidence for activation of the fetal androgen production in the setting of infection-induced pre-term labour, although many important questions remain. We present four cases with high testosterone level-associated pre-term labour. High testosterone levels associated with pre-term labour occurred in four pregnancies, providing one plausible cause of premature parturition.
