"P NMR proved to be a valuable research tool of human muscle bioenergetics in health' and disease.' Over the past five years, we have assessed its practical use in diagnosis and monitoring the course and therapy of myopathies. Abnormally low PCr/P, at rest (usually due to high Pi) was first observed in mitochondrial myopathies (MM)'-' indicating a reduced phosphorylation potentiaL6 Although 13 of 16 of M M patients showed this abn~rmal i ty ,~ a low PCr/P, a t rest was also noted in several destructive myopathies (eg. muscle dystrophy and polymyositis), in advanced atrophic disorders, and following acute muscle injury.? Secondary mitochondrial damage may exist in all these conditions, reflecting a final common pathway in diseased muscle cells.
