Techniques for repairing unilateral cleft lips have evolved continuously over the past few decades. Although many authors have revised the original Millard method,1–3 these methods have potential problems, such as obvious scarring along the nostril despite attempts to minimize the transverse scar on the alar base, and lip drooping caused by misapplication of small triangular flaps. Onizuka4 applied a small triangular flap above a white roll to allow enough rotation and to minimize the back-cut, whereas Noordhof and Chen5 avoided the alar base incision to prevent unacceptable scarring. Despite continuous efforts to improve unilateral cleft lip repair, few methods specific for incomplete cleft lip repair have been introduced.6 Unlike a complete unilateral cleft lip, the incomplete form has abundant tissue on the cleft side, especially at the nostril sill. Advancement of a lateral skin flap results in tissue redundancy at the columellar base and nostril sill that is then surgically excised to achieve a symmetric nostril. In contrast, redundant tissue is saved to augment the columellar base and nostril sill in our method. We believe that complete and incomplete cleft lips should be treated differently, which has led us to devise a specific method for primary repair of incomplete unilateral cleft lips. The present report describes a method where redundant tissue is saved to augment the columellar base and nostril sill at the cleft side, and the amount of skin flap paring and surgical incision is minimized.
