I thank Dr Tomoyuki Kawada for his interest in my review article, “Nutritional status related to poor health outcomes in older people: Which is better, obese or lean?”. He introduced important recent information regarding frailty and health risks in older people, showing that frailty increases the risk of all-cause mortality, and that the presence of frailty is not only associated with depression, but frailty is associated with the risk of onset of depression, and depression is also associated with the risk of developing frailty. He also reported that frailty and depression are risk factors for hospitalization. This letter by Dr Kawada reminded me of the close relationship between the nutritional status of older people and depression. My review mentioned above did not fully address the link between nutritional status and depression in older people. Needless to say, among psychological problems, depression can lead to malnutrition, and depression is also an important factor in malnutrition and weight loss among older people. Poor nutrition itself is also a risk factor for developing depression. There is a close causal relationship between frailty and malnutrition. In this way, both frailty and depression are not only involved in multimorbidity, which is an important clinical problem among older patients, but both pathological conditions are closely related to the nutritional status of older people. Therefore, the strong relationship between frailty and depression might be mediated by nutritional status. In other words, inadequate nutrition might be a factor in the development of both frailty and depression. I look forward to further developments in this field in the future.
