Complex Highways on the Translational Roadmap for Therapeutic Ultrasound-Targeted Microbubble Cavitation: Where Are We Now?

Therapeutic focused ultrasound offers a noninvasive approach to targeted delivery of ablative acoustic energy to structures within the body. High-intensity focused ultrasound is one form of therapeutic ultrasound that capitalizes on the conversion of focused acoustic energy into thermal energy to injure target tissues, such as currently used for cancer (1) and essential tremor (2). Harnessing the bioeffects of ultrasound for therapeutic purposes has more recently been explored in conjunction with diagnostic ultrasound contrast agents, which comprise gas-filled shell-encapsulated microspheres (microbubbles). Several of these microbubbles are approved by the Food and Drug Administration for use as red cell tracers for left ventricular cavity opacification during echocardiography, liver imaging, and evaluation of vesicoureteral reflux (3). Microbubbles compress and expand (oscillate or cavitate) in response to the cycles of ultrasound. In so doing, the microbubbles focus the energy of the ultrasound wave (wavelength on the order of millimeters) to a space on the order of magnitude of microns, leading to highly localized, often high, shear stress and/or microbubble destruction through inertial cavitation.
