During a post-mortem injection study of the human cardiac microcirculation, typical saccular and fusiform microaneurysms were observed in the heart of an insulin-dependent diabetic patient. To confirm initial observations, 5 additional diabetics were studied. These 6 patients were compared with 8 normoglycemic nondiabetic control subjects. Microaneurysms were observed in the hearts of 3 of the 6 diabetic patients. Microaneurysms evidently may be a more generalized feature of the diabetic microcirculation than has been previously recognized.
