The tumours most commonly reported to metastasize to the testis were prostate (45 cases, 35%), lung (25 cases, 20%), melanoma (12 cases, 9%), colon (11 cases, 9%), kidney (10 cases, 8%), stomach (6 cases, 5%) and pancreas (5 cases, 4%). The possible routes of spread from a primary site to the testis are (1) arterial; (2) retrograde venous; (3) retrograde lymphatic; (4) via the vas deferens, epididymis and rete testis; and (5) direct invasion. For primary tumours outside the abdominal or pelvic cavities, the arterial routes appear most likely, although metastatic involvement of intra-abdominal organs or lymph nodes may lead to testicular involvement by retrograde invasion through veins or lymph vessels. Once the testicular tumour is discovered to be a metastasis of malignant melanoma, the prognosis is very poor. In the metastatic testicular melanoma, the life expectancy after orchiectomy was from 2 to 14 months.
