Anesthesia for neurosurgical procedures requiring patient participation presents a demanding challenge to the anesthe-siologist. The patient must be alert, cooperative, and able to follow commands but also comfortable and immobile for long periods. These procedures include craniotomy for electrocorticography, mapping, and excision of epileptogenic foci, vascular malformations, and tumors involving vital cortical areas.
