METHODSBetween May 2005 and August 2013, we performed 39 right hepatectomies for large tumors. Attempts were made to apply the LHM at an early stage of the operation, ie, before liver mobilization. In the conventional LHM, a large vascular clamp was inserted from the bottom of the liver to its cranial side through the gap between the liver parenchyma and the IVC. If we experienced any difficulties in passing the clamp through the gap, we performed liver mobilization before the LHM. Since April 2010, we have used the saline injection method for the LHM in 12 consecutive cases. In the saline injection method, we set up an infusion system in which a 10-to 20-mL syringe was connected to a 100-cm infusion tube filled with saline. An 8-Fr stent drainage catheter (Cholangioentero-Tube, Create Medical Co Ltd) was then connected to the infusion system and inserted into the gap between the liver parenchyma and the IVC from the bottom of the IVC portal. As much saline as possible was injected during insertion of the catheter by hand injection. Typically, we used approximately 200 mL of saline for this technique. We kept injecting saline until the catheter had penetrated through the previously mentioned gap to the hole between the right and middle hepatic veins (video). The conventional method was applied in 27 cases, and the saline injection method was applied in 12 cases.
