Brown, MD, § and David Silver, MDv avid Hamerman, MD: As individuals age, they are in-D creasingly affected by diseases that result in mortality and morbidity. The conditions considered in this symposium are particularly representative of those that internists observe in their encounters with an increasing number of older patients, especially the “oldest old,”’and span the range from an acute fatal event, to an acute attack with associated morbidity, to only chronicity. This range is represented by the formidable toll imposed by ischemic vascular disease of the brain and heart and by osteoarthritis, the prototypical chronic, nonfatal disease of the joints. All these conditions result in widespread mortality and morbidity, impose high costs of care, and impair quality of life; thus, they are extremely important for consideration by healthcare providers in internal medicine and its specialties. In addition to their clinical importance, these conditions are also in the forefront of research relating to their basic pathophysiology. The first section of this Symposium introduces some of the newer insights in the fundamental mechanisms of the inflammatory process, including endothelial cell-leukocyte interactions, release of mediators, and tissue injury; these aspects are incorporated into the subsequent presentations of the disease states under consideration to permit more rational therapeutic interventions to limit damage in the brain, heart, and joint. Although not discussed in detail because of space limitations, the therapeutic modalities now emerging illustrate the best example of the translation of basic research to pharmacological and clinical collaborations, with the aim of extending the patient’s duration of life and improving its quality.
