Nuts are one of the most common foods causing allergic reactions in children and adults. Macadamia nuts are considered to be one of the prime edible nuts. These nuts originating from Australia have become popular in Germany since 2000. We report on 2 patients who developed an anaphylactic reaction with bronchial obstruction, urticarial rash and shock fragments after ingestion of macadamia nuts. Skin prick-to-prick test with macadamia nut was performed in the dermatological private office and revealed a strong immediate-type reaction to macadamia nut. Specific IgE antibodies to nuts could not be detected. A possible cross-reactivity with other nuts - particularly with hazelnut - is discussed, although there is no botanical relationship between the families of Proteaceae and Rosaceae. Until today, only 7 cases of an allergic reaction due to macadamia nut ingestion have been published in literature, mainly in Europe. The increasing import of these nuts to European countries, especially to Germany, might cause a larger number of hypersensitivity reactions in future.
