The etiology of type I, insulin-dependent diabetes mellitus (IDDM) is obscure; it may prove to have several different causes. A mononuclear cell infiltrate often is found in and around the islets of Langerhans early in the course of the disease. Later, a variety of histologic changes are found, but consistently the beta cell mass is decreased, and alpha and delta cells predominate in the islets [1], Dur ing the past 15 years, it has become increas ingly clear that both immunological and en vironmental factors are involved in the de velopment of the insular lesion of IDDM. The evidence for an immune pathogenesis for diabetes, while circumstantial, is compel ling. The nature of the environmental in fluences affecting the development of the dis ease remains to be defined. Although inflammation of the islets of pa tients with untreated ketonic diabetes was described as early as 1901 [2], Gepts [3] usually is credited with the detailed descrip tion of the lesion.
